Just realized the tile guy in Phoenix left a 3 inch gap behind the toilet flange and never said a word
Found it when I pulled the old toilet two weeks ago during my guest bath redo and now I need to know if I should fill that gap with thinset before laying new tile or if there's some reason he left it open.

Ugh, that is so annoying. He probably left it open because he was lazy and knew the toilet would hide it. You can fill that gap with thinset but make sure the flange still sits at the right height so the wax ring seals good. Some guys leave a gap for a reason, like if the flange is already too high and the tile would make it worse. But 3 inches is way too much to just ignore, that is not a small miss. Fill it in with thinset or even some patch concrete first, let it cure, then tile over it so you have a solid base.

Have you checked if there's a floor joist or plumbing running right under that gap? Sometimes guys leave a hole that size because they hit something when cutting and just said screw it. If its just open dirt or crawlspace under there you might be looking at air movement and moisture coming up through the tile later. Shine a light down in it and see whats under there before you fill it, because packing thinset over a void works but over a joist or pipe you need a different plan.
