Counted my scrap bin after forging 100 bottle openers and 23 of them ate it at the twist, what am I doing wrong

Man, I had the same thing happen with my first batch of fire pokers. Turns out I was twisting too cold, once I started doing it right at the forge with good orange heat my loss rate dropped way down.
