My domino joiner kicked a knot back at my face last Tuesday

I was out in the garage trying to cut mortises for a walnut bookshelf and this hidden knot flew off and hit me square in the forehead. Left a nice little dent and made me drop the whole project for the day while I sat there holding an ice pack and staring at my router. Has anyone else had a tool fight back at them like that or was I just asking for it?

Did you check the grain before you started cutting or just send it? My buddy Mike had a piece of oak literally crack a chisel in half and send a shard into his workbench last spring, he was just chopping dovetails for a chest and that knot had other plans. He said the chisel head went flying one way and the handle bounced off his safety glasses, which I guess is better than an eye socket. That walnut sounds like it had it out for you though, maybe it was just born mean.
